The following is the informed consent form that you must have on record with the Barrington Big Red, in order for your children to participate in the physical workouts at the Wauconda warehouse every week. INFORMED CONSENT FORM

I,_______________________enrolled in a program of strenuous activity including, but not limited to, weight training, sprinting, jumping, plyometrics along with throwing and hitting baseballs at the “Chicago Cutlery warehouse” located at 441 Bonner Road in Wauconda.  I hereby affirm that I am in good physical condition and do not suffer from any pre-existing condition or disability that would prevent my participation in this training program.

I fully understand the risk of injury from the training and conditioning involved in this program is significant, including the potential for permanent paralysis or death, and while particular rules, equipment, and personal disciplines may reduce this risk, the risk of serious injury does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the owners of Chicago Cutlery warehouse, L.B. Andersen and its lessors, and assume full responsibility for my participation.

I hereby release the owners and lessors of Chicago Cutlery warehouse at 441 Bonner Road in Wauconda, Illinois, from any liability now and in the future including, but not limited to, heart attacks, muscle strains, muscle pulls or tears, broken bones, shin splints, heat prostration, knee/foot/ankle/lower back injuries and any other illness, soreness or injury however caused, occurring during or after my participation in the training programs.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless the owners of the Chicago Cutlery warehouse and the lessors of the premises used to conduct any associated even {“releases”}, with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from negligence or the releases or otherwise.
